A crew personnel report (or crew report for short) was a standard report in Starfleet with the designation 15-5-01. It included details on crewmembers, such as their medical history, behavior on duty, and past performance.

Crew personnel reports might incorporate information from crew performance reports after they are filed.

Governments interacting within or with Starfleet were required to provide the medical history of people who become Starfleet crewmembers. Afterward, the medical history for Starfleet crewmembers was the responsibility of Starfleet Medical. The Doctor thought it was the agency's fault for not having a report for Kes on file to compare brain scans. (VOY: "Time and Again")
