A subspace anomaly was a very broad term for any sort of extraordinary disruption in subspace. Often, they could take the form of irregularities such as distortions, fractures, tears, or ruptures. Oftentimes they could damage equipment and personnel, altering physics and the Human brain.

In 2371, the USS Voyager was struck by a kinoplasmic radiation surge from a subspace anomaly. This caused The Doctor's program to malfunction, subjecting him to a delusion in which he was Human and Voyager was a hologram. (VOY: "Projections")
