A wrinkle was a fold of skin often present in old age. It could also refer to a fold in fabric. The noses of Bajorans were sometimes described as wrinkled. (DS9: "Waltz", "Chimera")

In 2371, while receiving a hug from Quark, Grand Nagus Zek told the bartender that he was wrinkling Zek's suit. (DS9: "Prophet Motive")

In 2372, the Kazon Kar asked Chakotay if he would rather die in his sleep as a wrinkled old man than be killed in battle. Chakotay agreed he preferred the former. Later, Kar admonished him to do what he said if he really wanted to die old and wrinkled. (VOY: "Initiations")

Some people believed that wrinkles were a sign of character, not ugly. Seska, upon seeing an older Chakotay, remarked on the truism that "Men just get more distinguished as they get older. A few lines here, a little grey there. It adds character. " (VOY: "Shattered")

A further meaning of the term was a complication or problem. A deleted scene from the Star Trek: Enterprise episode "Sleeping Dogs" had Phlox tell Jonathan Archer that he wished to share an "interesting wrinkle" involving an antitoxin he'd developed when Archer had the time.
